Welcome to Great Oaks Family Practice. It is our mission to provide you with the very best medical care and a superior level of personal attention. Dr. Diaz and his dedicated staff strive to make every patient encounter a comfortable, positive experience. We are accepting new patients on most major insurance policies (we do not, however, accept Medicare or Medicaid).
